Mileage Reimbursement Program

To get mileage reimbursement from Modivcare (formerly LogistiCare), you generally need to

follow a set process. Here is a basic outline of the steps.


Reminder we're located in the state of South Carolina. If you're located in a different state the companies and steps likely will be different.


Steps for Modivcare Mileage Reimbursement:


1. Eligibility:

○ Modivcare is for any type of insurance, Medicaid or Commercial.


2. Driver’s Profile

○ Before a trip, the diver must make a driver's profile by calling the reservation line.

(This line can give you more information regarding scheduling more than one trip

at a time.)

○ Reservation line for Horry and Georgetown Counties: 1-866-445-9954


3. Trip Number

○ A trip number is needed before the therapy appointment. You must call and

schedule the standing therapy appointment with Modivcare. This is the only way

a trip number can be obtained.


4. Physician/Clinician Signature

○ Your therapist must sign the authorized appointment as proof you were present.


5. Turning in the Form

○ Ensure the entire form has been completed by you, except the Physician/clinician

signature.

○ The form must be received within 30 days of the earliest appointment listed on

the form.

○ More than one therapy appointment can be listed on the form.

○ Only one driver can be on the form.

○ Once the form is complete, it can be mailed, faxed, or emailed.

Extra Tips:

Keep Copies: Always keep copies of submitted logs and any receipts or documents you

provide as evidence of your trip.


Submit on Time: Be mindful of any submission deadlines. Late submissions might

delay or disqualify your reimbursement.
